Pre-K Enrollment

INSTRUCTIONS FOR PRE-K ENROLLMENT

1. Complete the enrollment form at the button below.
2. Submit the following documents to the school (you may upload them in the enrollment form online or drop them off at the school).
   a. Child's Birth Certificate
   b. Parent's Driver's License/state id
   c. proof of address: current lease/rent receipt, utility bill or bank statement with your name and address on them
   d. Child's immunization records
   e. Completed Emergency Card 
   f. Early Learning Student Rights and Responsibilities- signed by parent
   g. Early Learning Discipline Policy- signed by parent
   h. permission to use technology
   i. work survey
3. Families are REQUIRED to submit an application for On My Way Pre-K voucher.

 

IMPORTANT INFORMATION FOR PRE-K PARENTS: 

• Please read this  letter from the school nurse  (Lea esta carta de la enfermera de la escuela).
• See the list of required immunizations.
• Contact your health care provider or the St. Joseph County Health Department (SJCHD) to schedule an appointment for these required vaccines. Immunization visits at the SJCHD are currently by appointment only. Call 574-245-6656 to schedule an appointment.
• Check out the Indiana Department of Education recommendations (Spanish version) for preparing your child for kindergarten.
